    What is Continuum of Care?

    Essentially, a system that provides a comprehensive range of healthcare services that evolve with the patient over time. A system that helps manage a patient’s health even at their most vulnerable with the least disruption to their quality of life.

    Achieving this goal requires a fundamental shift in thinking about how we design and deliver healthcare. Instead of thinking about healthcare as a series of episodic encounters with a provider, we need to think about it as a continuous relationship between the patient and the healthcare system.

    This shift in thinking is already underway in many parts of the healthcare system and is being driven by a number of factors. One is the increasing prevalence of chronic conditions, which require ongoing management over time. Another is the increasing use of electronic health records, which make it possible to track a patient’s health over time and locations.

The fee-for-service model incentivizes providers to do more, not necessarily better, and that is not a sustainable way to deliver quality care.

    The goal of value-based care is to improve patient outcomes, while also reducing costs. To achieve this, providers must work together to coordinate care across the continuum. This includes sharing patient information and collaborating on treatment plans. When everyone is on the same page, patients receive the best possible care and experience the best possible outcomes.
